ADMISSION PROCESS The applicant's primary care physician must complete a Medical Examination Form. An enrollment conference will be held with the applicant and caregiver to discuss policies, complete paper work, and obtain information about the applicant that will be helpful in designing an individual service plan. The applicant and caregiver visit Abundance of Love, tour the center and meet the staff. Application Forms Application for Enrollment and Medical information forms are available for referring agencies. Please complete forms and fax to 910-914-0711.
